MCQ 7

Which of the following cannot be assessed by an ultrasound?
a) placental age
b) fetal kidney function
c) fetal breathing
d) oesophageal atresia.

 Answer and explanation

Correct answer: B) Kidney function

An ultrasound uses the principle of sound wave transmission and reflection for visualisation of bone, soft tissue including muscle, tendon, vessels and even gas and fluid. In diagnostic ultrasound, sound waves of 2-18 MHz are used. In obstetrics the ultrasound can be abdominal or , as preferred, transvaginal. The latest type is a 3D colored Doppler ultrasound.

The placental site, and dimensions are made out in an obstetric ultasound and are co-related to the gestational age. Fetal breathing is seen as movements of the chest that appear as hiccups to the mother. Oesophageal atresia is suspected in prenatal ultrasound by a small or absent stomach or polyhydramnios (as the fetal is not able to swallow the fluid).

An ultasound is a valiable antenatal tool that helps in assessment of
>placental site and size
> no of gestational sacs ( eg. in multiple pregnancies)
>amniotic fluid volume,
>placental blood flow
>fetal heart rate
> any structural anomalies
>fetal movements and tone
> Tumours
> Pelvis

The BioPhysicalProfile ( BPP) is an important method of evaluation of fetla well being. It includes four US assessments and one Non Stress Test ( NST) to determine the following: Fetal heart rate, tone, fetal breathing, fetal movements, and amniotic fluid volume.

Fetal Kidney function, however, is assessed by amniotic fluid volume and fluid analysis. However, gross structural defects of the kidney can be made out on ultrasound.
